ПРОБИОТИКИ В ЛЕЧЕНИИ И ПРОФИЛАКТИКЕ ИНФЕКЦИОННЫХ ОСЛОЖНЕНИЙ НЕЙРОГЕННОГО МОЧЕВОГО ПУЗЫРЯ

Нейрогенный мочевой пузырь (НМП) – это дисфункция в работе мочевого пузыря, обусловленная врожденной или приобретенной патологией нервной системы, которая является распространенным патологическим состоянием и проявляется невозможностью осуществления произвольно-рефлекторного накопления и выделения мочи из-за органического и функционального поражения нервных центров и путей, что в значительной мере осложняет качество жизни пациентов. Нейрогенный мочевой пузырь часто сопровождается миофасциальным синдромом, синдромом тазового венозного застоя и инфекционными осложнениями.
Цель исследования. Оценить эффективность пробиотиков в профилактике симптоматической инфекции мочевого пузыря (ИМП) у больных с невропатией мочевого пузыря по данным, представленным в отечественной и зарубежной литературе.
Методы исследования. Проведен поиск литературных инсточников с 1993 по 2023 гг в базах данных CENTRAL, MEDLINE и EMBASE, материалов конференций, поискового портала Международного регистра клинических испытаний (ICTRP) рандомизированных контролируемых исследований (РКИ), посвященных использованию пробиотиков для профилактики ИМП у пациентов с нейропатией мочевого пузыря.
Результаты. Необходимым критериям РКИ соответствовали три исследования с участием 109 пациентов. В данных исследованиях изучалась эффективность введения пробиотиков в мочевой пузырь для создания безвредной колонии микроорганизмов в мочевыводящих путях у больных с дисфункцией мочевого пузыря.
Выводы. Показана относительная безопасность введения пробиотиков в мочевой пузырь для создания безвредной колонии микроорганизмов в мочевыводящих путях у больных с дисфункцией мочевого пузыря. Эффективность использования пробиотиков с целью предупреждения развития инфекционных осложнений у больных с нейрогенным мочевым пузырем не доказана. Необходимы дальнейшие тщательно спланированные исследования.
