САРКОПЕНИЯ У ПАЦИЕНТОВ С ХРОНИЧЕСКИМИ МИЕЛОПРОЛИФЕРАТИВНЫМИ ЗАБОЛЕВАНИЯМИ: КЛИНИЧЕСКИЕ И ФУНКЦИОНАЛЬНЫЕ АСПЕКТЫ

Аннотация

Саркопения – прогрессирующая потеря мышечной массы и функции, усугубляющая течение хронических миелопролиферативных заболеваний (ХМПЗ). Исследование направлено на оценку распространённости и клинико-функциональных особенностей саркопении больных с ХМПЗ. 
Цель. Оценить частоту встречаемости саркопении и провести сравнительный анализ клинических, функциональных и лабораторных показателей у пациентов с ХМПЗ в зависимости от наличия мышечного дефицита. 
Материал и методы. В исследование включен 51 пациент с ХМПЗ. Диагностика саркопении проводилась с использованием опросника SARC-F, функциональных тестов (SPPB, динамометрия, тест ходьбы) и лабораторных анализов. 
Результаты. Саркопения выявлена у 17,6% пациентов. Для данных пациентов зафиксированы: более старший возраст (70,1 против 58,1 лет; p<0,001), повышенный индекс коморбидности Чарлсона (6,1 против 3,6; p<0,001), снижение физической работоспособности (SPPB: 5,3 против 9,1 баллов; p<0,001) и переносимости нагрузки (дистанция ходьбы: 172,8 против 263,7 м; p=0,001), а также более низкие показатели гемоглобина (126,4 против 146,2 г/л; p=0,016) и гематокрита (38,9% против 44,1%; p=0,038). 
Заключение. Саркопения ассоциирована со значительным снижением функционального статуса у пациентов с ХМПЗ, что требует её раннего выявления и коррекции.
